Inner surfaces of the limbs like that suggests either systemic illness or food allergy rather than a response by the skin to some external factor. It can't hurt to try an elimination diet (based around lamb, rice and vegetables, eliminating wheat, dairy, eggs and nightshades). Look for Jonathan Brostoff and Linda Gamlin's book about how to do it and interpret the results. There is no risk of malnutrition with this (might well be better than the diet he has now) and if it works you should know in a month.

I used to use coal tar soap. It's now nearly impossible to get hold of. Wright's (formerly the leading brand) no longer contains any real coal tar - the smell is entirely simulated with chemicals which are likely to be allergenic in their own right. In any case, external treatments like that are not as effective as dealing with the real cause, if it's a dietary factor or a side-effect of some other illness.
